Interview patient, collect data and compile an accurate medication history for patients being admitted to hospital or seen in the emergency department. Document and review all medication orders in history to prepare for provider to reconcile.

Work Hours/Shift:

  • full time 40hr per week, rotating days, evenings and weekends (differentials apply!)

You Will Be Responsible For:

  • Exceptional Patient Care: Perform medication histories on patients admitted to the hospital through the Emergency Department

  • Excellent Communicator: Maintain regular communication with admitting providers, Emergency Department nursing staff and Emergency Department provider staff

  • Strong Educator: Contribute to program optimization and provide feedback to Lead Medication History Technician and/or supervisor as needed, or as requested

  • Technical Familiarity: Utilize the hospital’s electronic health record system to document the patient’s current medication list into the electronic medical record

Requirements:

  • High school graduate or equivalent. Completion of a hospital-based, community college or military technician training program preferable

  • State Pharmacy Technician registration must be current; or registration as a Pharmacy Intern and actively enrolled in a school of pharmacy program.

Preferred Skills: ​

  • PTCB National Certification (CPhT) desired, not required

  • Two years hospital or retail pharmacy experience preferred
