The Ambulatory Pharmacy Liaison provides for improved service levels for Hackensack Meridian Health (HMH) patients by seeking to supply medications prescribed by the patient's physician for discharge in a timely manner so that the patient can be discharged efficiently from an acute care HMH facility and transition directly to home. Assists with obtaining information necessary for prescription processing, helps prioritize workflow and facilitates delivery of completed prescriptions back to the patient prior to discharge. Ensures patient medication educational needs are met. Responsibilities include the job functions of an Ambulatory Pharmacy Technician as needed / required, but follows a unique job description.

Responsibilities

A Day in the Life of an Ambulatory Pharmacy Liasion at Hackensack Meridian includes:
Establish an initial greeting to patients admitted and explain services available via the HMH Pharmacy.
Works with floor nurses, discharge planners, physicians, unit clerks and other teams/initiatives as required to determine patients being scheduled for discharge.
Obtains patient authorization to process discharge prescriptions through the ambulatory pharmacy.
Determines information sources necessary to process prescriptions.
Adheres to all established HMH Health and Federal/State privacy and compliance policies to protect patient confidentiality.
Transmits required information to pharmacy and ensures prescription fulfillment based upon priority needs to allow for timely discharge.
Coordinates and facilitates patient pickup of prescriptions and/or delivery of completed prescription orders to patient care areas.
Ensures patients receive offers for pharmacist counseling and distributes pharmacist contact information in the event patient has prescription-related questions at home.
Facilitates distribution of medication-related educational materials and coordinates counseling with staff pharmacists where needed.
Assists Ambulatory Pharmacy staff as needed.
Performs duties of Ambulatory Pharmacy Technician as needed / required.
Ensure all guests/patients receive safe care. Maintain a "safety first" work ethic. Report any medication errors or medication variances as required by HMH Policy.
Other duties and/or projects as assigned.
Adheres to HMH's Organizational competencies and standards of behavior.
Lifts minimum of 10 lbs., pushes and pulls minimum of 40 lbs. and stands minimum of 4 hours a day.

Qualifications

Knowledge, Education and Skills Required:
High School Diploma or equivalent.
Ambulatory experience with knowledge of medications.
Knowledge, Education and Skills Preferred:
One or more years of experience in a retail or hospital based ambulatory pharmacy.
Required Certification/Licensing:
NJ Registration as Pharmacy Technician.
Preferred Certification/Licensing:
Valid Certification from PTCB or equivalent.
Certification, acquired through a NJ Board of Pharmacy Approved testing agency.
