The Pharmacy Resident is a graduate pharmacist enrolled in a one year program that provides further training in comprehensive hospital pharmacy practice with an emphasis and focus on the clinical aspects of pharmaceutical care.
A day in the life of a Pharmacy Resident at Hackensack Meridian Health includes:
Complies with all state and federal regulations and Medical Center policies and procedures relating to drug dispensing, including appropriate handling of drug products, adherence to security procedures, and completion of necessary documentation. This includes appropriate control and dispensing of controlled dangerous substances and investigational drugs.
Provides staffing coverage as dictated by the goals of the residency program. This includes medication order review and dispensing, and the preparation of admixtures using aseptic technique (e.g., IV solutions, TPN, chemotherapy).
Maintains competency in clinical aspects of drug therapy in order to assist physicians and nurses in providing appropriate pharmaceutical care.
Participates in patient care rounds by providing comprehensive pharmaceutical information relating to the care of patients.
Monitors drug therapy for appropriateness, including indication, use, dose, dosage form, route, possibility of therapeutic duplication, adverse effects and drug-drug/drug-food interactions.
Provides pharmacokinetic consultations as required.
Precepts pharmacy students in conjunction with the Clinical Pharmacy Staff.
Conducts targeted drug usage evaluations as required by residency goals.
Reviews physician orders for possible therapeutic problems, contraindications, interactions, allergies, formulary status and clarifies all areas of uncertainty.
Counsels patients on drug therapy in accordance with residency goals.
Employs principles of safe medication practice as dictated by departmental and organizational policies and procedures.
Identifies the needs of the patient population served and modifies and delivers care that is specific to those needs (i.e., age, culture, language, hearing and/or visually impaired, etc.) This process includes communicating with the patient, parent and/or primary caregiver(s) at their level (developmental/age, educational, literacy, etc.).
Adheres to the standards identified in the Medical Center's Organizational Competencies.
Education, Knowledge, Skills and Abilities Required:
Graduate of an ACPE-accredited school of pharmacy, or equivalent, with a minimum of a Pharm. D degree.
Education, Knowledge, Skills and Abilities Preferred:
Knowledge of hospital pharmacy practices, including: unit dose dispensing, and principles of aseptic technique.
Knowledge of word processing and spreadsheet software.
Licenses and Certifications Required:
Licensed as a pharmacist in the State of New Jersey or eligible for licensure as a pharmacist in the State of New Jersey. If not licensed, the candidate must become licensed during the residency program in order to fulfill the staffing component.
