Wewant your knowledge and expertise!Here is some of what you can expect from being a Pharmacy Clinical OperationsSpecialist with us:Interpret orders and transcribes or verifies them accurately and efficientlyinto the pharmacy computer system.Accurately compound and dispense pharmaceuticals.Complies with facility policy and procedures and regulatory requirements.Monitor drug therapy regimens for contraindications, drug-druginteractions, drug-food interactions, allergies and appropriateness ofdrug and dose.Provide clinical consultation and clarification to practitioners. Suggestappropriate, cost-effective therapeutic alternatives to medical staff asneeded.Sustain the hospital formulary, minimizing non-formulary procurements,utilizing therapeutic interchanges and promotes rational drug therapyselectionAttend and actively participate in daily multidisciplinary and bedside rounds.Complete drug utilization reviews to promote rational drug therapy andimplement improvements identified.Provide accurate and timely medication information to healthcare staff andpatients.Lead and develop pharmacy clinical services such as drug consults, nutritionsupport, pharmacokinetics, anticoagulant monitoring, pain management,IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.Develop, manage and mentor antimicrobial management program (AMP).Document all clinical activities and interventions accurately and completely.Responsible for mentoring and education as well as ongoing analysis ofinterventions to insure accurate and appropriate documentation and providesummary reports.Support and participate in the goals of the organization's qualityimprovement, risk management and patient safety program to include but notlimited to National Patient Safety Goals, Core Measures and HCAHPSmedication related initiatives.Participate in the design, collection, and reporting of adverse drugreactions, drug utilization reviews, medication errors and pharmacyquality initiatives.Lead the training and mentorship of pharmacists, pharmacy students,residents, technicians, nurses and physicians. Develop and document staffcompetency on an ongoing basis and develops improvement plans whendeficiencies in competencies are identified.Lead in preceptorship of pharmacy students and/or residents.Maintain professional competency sufficient to meet current and future needsof the facility.Provide and demonstrate expertise in a specialized area of pharmacy practice(operational
